The Allergies page displays a table of the patient's self-entered allergy and drug sensitivity information. The table contains the following fields: Date, Allergy, Severity, Reaction, and Comments. The patient and/or the patient's delegate(s) can make all changes to this information. The patient's grantee(s) can only view all fields, if he/she has been given access to this page.
| 1. Click the Add New Record button. |
| 2. In the Date field, enter the date of the allergy or drug sensitivity or select a date from the calendar by clicking the icon and choosing a date. |
| 3. In the Allergy field, enter the name of the allergy or drug sensitivity. |
| 4. In the Severity field, enter the severity of the allergy or drug sensitivity. |
| 5. In the Reaction field, enter the reaction to the allergy or drug sensitivity. |
| 6. In the Comments field, enter details, remarks, comments or notes concerning the allergy or drug sensitivity. |
| 7. Click the Save button. |
When editing, all fields do not have to be updated, only those that require a change.
| 1. Click the Edit button next to the record you wish to edit. |
| 2. In the Date field, enter the updated date of the allergy or drug sensitivity or select it from the calendar by clicking the icon and selecting a date. |
| 3. In the Allergy field, update the name of the allergy or drug sensitivity. |
| 4. In the Severity field, update the severity of the allergy or drug sensitivity. |
| 5. In the Reaction field, update the reaction to the allergy or drug sensitivity. |
| 6. In the Comments field, update details, remarks, comments or notes concerning the allergy or drug sensitivity. |
| 7. Click the Update button. |
| 1. Click the Delete button next to the record you wish to delete. The selected allergy or drug sensitivity is deleted from the table. |
